It appears to me, that one of the tapered front sight base pins could be at fault (it could be both pins if they were not installed correctly) a gunsmith who is familiar could fix this problem
If this base came from another rifle, it probably will give you that symptom. I found swapping around FSB wasn't the best idea. Others here have disagreed, saying if they were milspec, they'd be OK. Not my experience though. But as RCS says...maybe the pins are coming in from the wrong side.
To the best of my knowledge the fsb is original, and the barrel also .

It must have been removed, they don't lean from the factory. I did however once buy a new flattop receiver that had the top out of whack. It leaned a bit and iron sights were impossible. Changed to a Cerro forge done by CMMG and the problem was solved. I sold the wacky upper...
Years ago there were some original 20" USGI barrels showing up at the gun shows still wrapped in the foil paper and all of them had the gas block/sight base off about 3/16" or more. This was way before the internet and I have no idea what the outcome was. I went through three before I got one straight. One of the gun show regulars named Dirk Seely had some.
I think Bill Hollinger may have the answer - a bent barrel ! I seem to remember that there was a gage that went inside the bore to detect a bent barrel ?
I have one here. It's about 6" long, in the white and hardened. Just drop it through...if it catches at all, there's something amiss.

This is a DSA lower and the fella called me last night and DSA said they will make it right.
Oh well , we will see.
The FSB is located by the tapered pins, it may have been reamed offset. You can buy the reamer from Brownell's as a gunsmith tool $$$ or from a machine shop supply as a 0/2 taper reamer for $. You may be able to correct this by reaming and installing new taper pins, possible a new FSB also.
You could replace the FSB with a clamp on model with set screws.

It's not that uncommon to find canted front sight bases on AR and AK barrels. I've repaired them in the past by resetting them, (usually on the range at 25 yards), reaming again, and then replacing the pins. Another very helpful tip I was taught is to run some soft solder down the holes too after reaming. Then hit the holes one last time lightly with the taper reamer. This will keep the front sight base from shifting back to the original position when you drive the taper pins in.
The rifle is en route too DSA.

They'll sort it out.
Rifle has returned to owner from DSA.
I have not seen it as yet maybe next week.
He must have looked at it though. I'm sure it's fine now. I'm surprised it escaped in the first place but if it's a flat top, they might not notice.

Saw the gun today and looks good and shot fine.
DSA changed out the bbl and receiver @ N/C
