    Out-of-battery ignition? She did kinda ease the bolt closed as opposed to just letting it fly shut. Or over-charged hand-load?

    It's an odd failure. Some insight by Gus Fisher, et al. But without detailed photos it seems to be either a case failure or some sort of unlocked bolt discharge. (Which is exceedingly hard to do if everthing is working properly!)
