It is a special multi-purpose tool for doing vasectomies and circumcisions. There is a smaller version for ear and other body piercings

What happens when the handles are squeezed together? Is it a tool which crimps something? What is the configuration of the hole in the top--does it go all the way through that top part? Would it have something to do with leather work or saddlery, or perhaps loading cartridges? Hard to tell from the photos..... Interesting old piece.
