And bad spot in the forging that was deemed Ok by inspection. That is not a high-stress area. You'd be surprised at some of the receivers that passed inspections, especially the IHC's.

photos show a early four digit SA receiver with a small defect on the receiver ring
On Mark's photo, sometimes an out of battery firing of the cartridge while being chambered can
chip the receiver like the one in the photo

Yes, it looks like a chip more than a flaw.
