This seems to be a long-standing interest of yours? I don't think you are going to find any such photos, but there isn't really any need for them: a standard 3/8 Winchester/Lyman dovetail block at the rear, and the same at the front with a "flat bar" extension added, or a dovetail block machined with such a riser as one piece. Screw and solder down and there you are. Of course if you want the graduations on the adjustments to have their full range of travel, you'll need to collimate the bases and rings with the appropriate degree of declination. If you buy the reprint of the factory brochure on the A5 it gives more details.
If you want a tip on how to collimate the rings to the bore, my advice would be to pick up a surplus bore sight scope and adapt that into the rings with suitable spacers.
