Lead bullets? If so, the root of your keyholing problem could be in their composition and sizing combined with a rough bore.
You might find some jacket fouling locked into the bore now. Could be it built up enough to squeeze those last bullets down enough that they weren't contacting the bore. I've done that with lead...
