    a loose or too tight action wrench, using a tool thats NOT a action wrench as an action wrench,
    tightening the wrench down on the ring, then pressure will crack a ring, uneven pressure, and so on.
    most action wrenches have a step, that usually mates with the step on the right side of the receiver ring,
    if you install the step on the other side, it will put spot pressure on the smooth edege, as soon as someone put some gorilla pressure..snap.
