Now I feel like a kid in class at the chalkboard and I just added 2 plus 2 and came up with 5 in front of everybody. I've double checked all the boxes and can't see a mistake anywhere. Did I miss one of the numbers or the letter at the end? Spill the beans boyz. What do you see?

Oh, did you mean mark the slide type boxes with a checkmark? I didn't mark them since the number/letter in the corresponding box is used to form the code to place in the empty space under SLIDE. Ted from CC either missed it or I was correct to not mark them.

Do I need to check mark them and send along another copy?

I have had nought to do with that type of form but have filled out a few others and honestly with a busy page like that your bound to be missing the forest for the trees bit like a dangerous goods audit for storage and compliance I did by myself at the power station I worked at.
I can see boxes that are not ticked so am taking it that the marks are not there you do not tick it, though I did notice the recoil plate and recoil screw bottom right is not ticked

My recoil plate has no marks and the recoil plate is different for the M1A1. Neither screw is applicable, hence no mark.

I believe both the recoil plate and recoil plate screw "boxes" are meant only for the M1. I usually put (4) red dashes (hyphens) in boxes where I have no input, to indicate those boxes weren't overlooked. It is a very "busy" form! I have an Inland M1 with a s/n in the 400K range, and just finished comparing my data sheet with HDSledge's. A lot of the choices made when filling out these forms are subjective in nature. I marked my dogleg hammer as "white" (no finish) but it could be blued. I questioned the shape of his "N" front sight but went back, took a look at his pictures, and agree. The unmarked handguard - not sure on that one. I do see discrepancies with the slide stop, barrel skirt, slide type, and mag catch. Not all are pictured, but some parts on his M1A1 appear to be later designs than the ones on my M1. But, I've learned there are reasons why that sometimes happened - I do have to admit that's the nicest looking "Paratrooper" I've ever seen - as well as excellent photos! - Bob

I took it to a friend who collects to look at and he wouldn't stop holding it. I was afraid he was not going to let me have it back.

Data Sheet: CrossedCanons and ISGI are correct. The boxes are indeed supposed to be checked on the slide section according to the archivists. The example they sent me shows the boxes filled in obscuring the letters.

They also pointed out the handguard. I couldn't find a noticeable mark anywhere on it so I left it blank. When I have it out in the spare stock I will look again. I read somewhere that not all handguards have a mark. (uscarbinecal30?) I'll try some contrast lighting and filters within photoshop to see if I can spot a mark.

Re. slide stop, barrel skirt, slide type, and mag catch. My slide stop has a detent area like the one in selection 2. Drawing does not seem to be to scale though on the dimensions. My mag catch is definitely the type 2. The inventory sheet for my serial number indicates a type 2 slide, a type 2A mag catch and either a type 2 or 3 slide stop.

What is a type 3 slide stop? It is not on the sheet as a choice. What is the type 2A mag catch? Also did you mean chamber skirt? Mine is definitely the short one.
My arm joint is a type 1 not a type 2.
