Fortunately a "controlled explosion" wasn't carried out "in situ".

The mind boggles as to how this accident could have occurred?

It was a total dis-***-ter, doctor. A real cat-***-strophe. I was in the rear with the gear, and now I have some major disbumfort. I would have come right away, but the Mrs. wasn't cheeky about me driving myself in this condition and didn't want to be party pooper.
