It is a special multi-purpose tool for doing vasectomies and circumcisions. There is a smaller version for ear and other body piercings

You say it's a multi-tool, wot else is it used for?

What happens when the handles are squeezed together? Is it a tool which crimps something? What is the configuration of the hole in the top--does it go all the way through that top part? Would it have something to do with leather work or saddlery, or perhaps loading cartridges? Hard to tell from the photos..... Interesting old piece.

I find it hard to explain the operation of this tool in your language.

More pictures to understand the movement

That's confirmed it. Vasectomies and circumcisions with a bit of light body piercings at the weekend
