    Machining run out. Likely not in the jig properly.
    Possibly the machine operator fed the bit too fast.

    I have seen a few SA slides like that.

    That doesn't seem likely to me since the machined hole looks round and the wall doesn't appear thin at the "breakout." If true, wouldn't it have been scrapped on the production line or at QC inspection? - Bob
