What would cause this failure to feed?

Perhaps the chamber needs a bit of paper polishing on a mandrel? Perhaps the mag catch is allowing the mag to sag down? Hand feeding is never a good measure of how a weapon works though. Try holding the mag up when operating or firing. See if that sorts it out?
